Desert Sands Unified School District is self-insured for workers' compensation. Keenan & Associates is the district's claims administrator. This means all medical and other costs associated with work injuries are paid directly by the district's general fund.
Employees must report all accidents, injuries, and near accidents/injuries immediately to their supervisor. The supervisor is required to complete an injury report for all workplace injuries and forward the report to Risk Management. The injured employee is responsible for submitting a copy of each and every physician work status note after each appointment. All physician and physical therapy appointment dates and times are required to be reported to your site and to Risk Management. It is recommended that all appointments be scheduled after your normal work hours.
If an employee requires medical treatment for a workplace injury or illness, they should call the Company Nurse Hotline at 1-877-518-6702 and reference code QS729. The employee is also required to contact Risk Management after the physician visit to obtain additional information and treatment authorization forms.
If an employee will not be seeking medical treatment, but would like to use a "Report Only" form to report the incident, they should visit: https://www.dsusd.us/wcinjury and fill out the Google Form.

You will be asked to show your District identification. In the event of a serious work injury, the employee should be seen at the nearest hospital emergency room and Risk Management should be notified immediately.
